Venous duplex ultrasound is a vital tool in the diagnosis and treatment of varicose veins and spider veins. During your initial vein consult, Dr Ryan McConnell will perform a detailed ultrasound of the veins within the leg of concern. The ultrasound is non-invasive, evidence-based and takes approximately 40 minutes.

What does the ultrasound assess?

The venous duplex ultrasound allows for an accurate assessment of varicose veins in order to guide treatment planning if required. It assesses:

  • Whether the deep veins of the leg are normal.
  • If there is venous reflux. If reflux is present, the source of reflux may be assessed.
  • The path of venous reflux including the vein branches affected.
  • If there are any obvious problems which would prevent endovascular treatment.
  • The location of the refluxing vein with respect to the skin and surrounding structures.

Do I need an ultrasound for spider veins?

As spider veins on the legs are often associated with underlying venous reflux, it is important that a venous duplex ultrasound be performed. This will allow the underlying venous disease to be detected and treated so that when the spider veins are treated there is less chance of recurrence. Failure to detect underlying venous reflux may contribute to spider vein matting at a later stage (a cluster of extremely fine spider veins).

How is it performed

  1. The venous ultrasound takes approximately 40 minutes.
  2. The best examination of the leg veins is performed when the veins are distended (you will be advised to not wear stockings on the day of the examination).
  3. The ultrasound is performed with the patient upright, standing on an elevated platform. This allows blood to pool within the veins of the legs and the ultrasound scan is more likely to detect venous reflux if present.
  4. Images of the deep and superficial veins within the leg will be obtained (the scan extends from the groin crease to the level of the ankle joint).
  5. An assessment for venous reflux will be carried out by the sonographer performing several augmentation manoeuvres. This often involves a squeeze of the leg below the vein segment being imaged.

Ultrasound results

Dr McConnell is an Australian trained interventional radiologist with extensive training and experience in image interpretation and endovascular procedures. Dr McConnell will provide an accurate written and graphical report outlining the ultrasound findings. He will explain to you the ultrasound findings so that you are completely informed of the degree of vein disease. The ultrasound will be used to help provide the optimal treatment plan for the patient’s particular venous condition.
